Preparedness levels have been shown to improve the outcomes for people who find themselves in an emergency. However, uptake of preparedness behaviors by the public prior to a major disaster is limited. This 2-part study examined perceived preparedness in the UK during the first months of the COVID-19 pandemic (Study 1), and 2 years later (Study 2).

Both studies investigated the effect of individual demographics (gender, age, perceived socioeconomic and health status) on perceived preparedness. Next, the studies examined the extent to which perceived preparedness was associated with mental health outcomes (anxiety, depression, and stress symptoms). Participants (Study 1, N = 409) completed an online survey in May to June 2020 during a national lockdown, with another sample (Study 2, N = 87) completing the same survey from March to July 2022.

Across both studies, participants completed 2 to 3 different preparedness activities. Greater subjective perceptions of socioeconomic status were associated with perceived preparedness. Preparedness levels were related with better mental health, and unrelated to age and gender.

Encouraging the public to engage with preparedness behaviors may not only have practical benefits but also help to protect mental well-being during a disaster.

This study investigated the psychological and physiological effects of using sleeping bag-shaped aluminum sheets during night-time sleep in winter to reduce cold stimulation and improve the sleeping environment in disaster evacuation shelters.

Sixteen healthy male participants in Hiroshima City underwent 2 experimental conditions in January and December 2023: night-time sleep in winter with and without a sleeping bag-shaped aluminum sheets. Sleep–wake rhythm during night-time sleep (discriminant analysis method), subjective sleep soundness, mood state profile, heart rate, blood pressure, and cardiac autonomic nervous system modulation were measured. Wilcoxon’s signed-rank sum test, Friedman’s test, and paired t-test were performed.

Thermal sensation of warmth and subjective sleep soundness were significantly greater with than without the aluminum sheets. Total sleep time and sleep efficiency were also significantly greater, whereas the frequency and duration of wakefulness after sleep onset were significantly less. Total mood disturbance, fatigue-inertia, and vivid-activity were significantly different between the control, aluminum sheets, and home conditions. Physiological parameters did not differ significantly.

These data suggest that using sleeping bag-shaped aluminum sheets in disaster evacuation shelters in winter could be effective in improving sleep–wake rhythms and subjective sleep soundness, although negative emotions were not improved.

The catastrophic Beirut blast on August 4, 2020 at 6:07 pm resulted in extensive damage. Our study aims to categorize the injuries of patients who were transferred to the radiology department in the first 12 hours following the blast and to evaluate the disaster preparedness of the radiology department at Hôtel-Dieu de France Hospital in order to implement corrective action process.

A total of 97 patients underwent imaging examinations, comprising 77 CT scans and 20 radiographs, which were retrospectively reviewed by 4 senior radiology residents. Patient injuries were classified according to blast injury categories. A full assessment of our disaster plan was done including staff shortage, examination time, patients triage and diagnostic performance of radiologists.

Tertiary injuries were the most prevalent (47.6%). Maxillofacial fractures and intracranial hemorrhage were the most common (54.3%). The radiology department encountered numerous challenges, including communication difficulties, staffing shortages and infrastructure damage. Nevertheless, the disaster management plan enabled the department to effectively navigate these obstacles despite many flaws and many lessons were learned.

This study underscores the importance of an efficient response of the radiology team during mass casualty incidents and can inform future disaster preparedness efforts in health care settings.

The scoping review aims to provide an overview of the existing literature to inform an understanding of pharmacists’ roles, skills, and knowledge requirements for Emergency Medical Teams responding to disasters or humanitarian crises.

The methodology utilized the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR) guidelines, with methodology adapted by the Joanna Briggs Institute. Six databases were searched for sources published after 2000: PubMed, Mednar, Scopus, Defense Technical Information Centre, LILACS, and CINAHL. A manual search of grey literature was conducted to discover additional sources of information outside of the electronic databases.

Of the 427 initial studies and reports, 36 were selected for inclusion. Five key themes on role requirements were identified: supply chain management; establishment and operationalization of pharmacies; stakeholder communication, collaboration, and liaison; direct patient care; and response management and coordination. Each theme was delineated into subthemes linked with requisite skills and knowledge.

This scoping review identifies key roles, skills, and knowledge requirements of pharmacist contributions to Emergency Medical Teams responding to disasters or emergencies.

National Health Service (NHS) England conducts annual assurance of NHS bodies in England’s readiness to respond to emergencies using its Core Standards for emergency preparedness, resilience, and response (EPRR). This review assessed whether the first complete EPRR assurance after England’s coronavirus disease (COVID-19) pandemic national response was performed successfully.

The primary outcome of interest was the quantity of information regarding applicable Core Standards held by NHS England at the end of that assurance. Secondary outcomes were variations between the number of applicable Core Standards and information held by NHS bodies about the number of applicable Core Standards.

NHS England recorded the correct number of applicable Core Standards for 88 of the 124 NHS trusts in England which provided general hospital accommodation and services in relation to accidents or emergencies. It recorded an incorrect number of standards for 13 trusts and did not record the number of standards for 23 trusts.

NHS England’s EPRR assurance resulted in correct data not being recorded for over a quarter of the above NHS trusts. This review may also be of interest to other state-level bodies that rely on the high-level assurance of their ability to provide health care during emergencies.

Chemical, biological, radiological, and nuclear (CBRN) events with multiple casualties are rare events, but preparedness is crucial for hospitals to respond properly. This study evaluated the preparedness and disaster planning of German hospitals for CBRN incidents.

In a cross-sectional study, German hospitals with level III (highest level) emergency departments were surveyed using an online questionnaire focusing on risk assessment, infrastructure, hospital disaster planning, and preparedness for CBRN events.

Between June and July 2023, 50 hospitals were surveyed. 62.5% of the hospitals had a section on chemical incidents in their disaster plan. A decontamination facility was available in 29.8% of the hospitals and chemical protective suits in 46.8%. The minority of the hospitals trained the correct handling of personal protective equipment (PPE) (39.1%) regularly or had frequent CBRN drills (21.3%). Most hospitals had the infrastructure for medical isolation (93.6%).

The level of CBRN preparedness is heterogeneous for German hospitals. Most were well prepared for infectious patients, but only half of all hospitals had sufficient PPE for chemical incidents and only 30% had a decontamination facility available. Overall, the level of CBRN preparedness is still insufficient and needs further improvement.

Mass Casualty Incidents (MCIs) involving high-speed passenger ferries (HSPFs) may result in the dual-wave phenomenon, in which the emergency department (ED) is overwhelmed by an initial wave of minor injuries, followed by a second wave of more seriously injured victims. This study aimed to characterize the time pattern of ED presentation of victims in such accidents in Hong Kong.

All HSPF MCIs from 2005 to 2015 were reviewed retrospectively, with the time interval from accident to ED registration determined for each victim. Multivariable linear regression was used to identify independent factors associated with the time of ED presentation after the accidents.

Eight MCIs involving 492 victims were identified. Victims with an Injury Severity Score (ISS) ≥ 9 had a significantly shorter median time interval compared to those with minor injuries. An ISS ≥ 9 and evacuation by emergency service vessels were associated with a shorter delay in ED arrival, whereas ship sinking, accident at nighttime, and a longer linear distance between the accident and receiving ED were associated with a longer delay.

The dual-wave phenomenon was not present in HSPF MCIs. Early communication is the key to ensure early resource mobilisation and a well-timed response.

In 2020, Japan’s Ministry of Health, Labour and Welfare developed an Excel workbook entitled “Simple Simulator for calculating nutritional food stocks in preparation for large-scale disasters.” In September 2021, it was modified as the “Revised Simulator” to plan food stockpiles in normal times and post-disaster meals. This study aimed to further improve the Revised Simulator.

Eight group interviews were conducted with 12 public health dietitians, 9 disaster management officers, and 2 public health nurses from September to November 2021. They provided nutritional support during previous disasters or prepared for predicted future disasters. Qualitative analysis was conducted on interview transcriptions, then the Revised Simulator was improved based on their feedback.

The Revised Simulator was improved to the “Simulator for calculating nutritional food stocks and meals for large-scale disasters” with significant changes such as adding specific tags in the food list to denote long shelf life and elderly-friendly foods, as well as displaying bar graphs to visualize the required and supplied amounts of energy and nutrients.

The Revised Simulator was upgraded for planning and assessing stockpiles and meals in ordinary conditions and emergencies. This study will contribute to enhancing the quality and quantity of food supplies during disasters.

This work aimed to demonstrate that a website, www.epidemic-em.org, encompassing “static” resources, and videos, as well as other tools, can be used to strengthen public health emergency management capacity during epidemic response.

Existing resources were updated and developed for self-directed Emergency Operations Centers’ capacity strengthening, in order to encompass current best practices, and to emphasize how public health emergency management concepts can support epidemic response activities. These materials formed the core of the website, launched in June 2020, to which country case studies were added. In 2021, a pilot virtual training program was designed using recorded video lectures and interviews with global experts in addition to the website material, which was delivered to South African responders.

The website has been accessed in more than 135 countries, demonstrating widespread reach and interest in online and freely accessible materials to support public health emergency operations. Over 30 people participated in the pilot virtual training, and the evaluation showed improvement in knowledge, confidence in using emergency management concepts for epidemic response, and positive feedback on the virtual modality.

Online tools can expand access to materials and resources for public health emergency management capacity strengthening. Virtual modalities can further serve as a powerful complement, and perhaps replacement, for traditional in-person technical assistance, despite some limitations.

As disasters are rare and high-impact events, it is important that the learnings from disasters are maximized. The aim of this study was to explore the effect of exposure to a past disaster or mass casualty incident (MCI) on local hospital surge capacity planning.

The current hospital preparedness plans of hospitals receiving surgical emergency patients in Finland were collected (n = 28) and analyzed using the World Health Organization (WHO) hospital emergency checklist tool. The surge capacity score was compared between the hospitals that had been exposed to a disaster or MCI with those who had not.

The overall median score of all key components on the WHO checklist was 76% (range 24%). The median surge capacity score was 65% (range 39%). There was no statistical difference between the surge capacity score of the hospitals with history of a disaster or MCI compared to those without (65% for both, P = 0.735).

Exposure to a past disaster or MCI did not appear to be associated with an increased local hospital disaster surge capacity score. The study suggests that disaster planning should include structured post-action processes for enabling meaningful improvement after an experienced disaster or MCI.

Under-resourced communities face disaster preparedness challenges. Research is limited for resettled refugee communities, which have unique preparedness needs.

This study aims to assess disaster preparedness among the refugee community in Clarkston, GA.

Twenty-five semi-structured interviews were completed with community stakeholders. Convenience sampling using the snowball method was utilized until thematic saturation was reached. Thematic analysis of interviews was conducted through an inductive, iterative approach by a multidisciplinary team using manual coding and MAXQDA.

Three themes were identified: First, prioritization of routine daily needs took precedence for families over disaster preparedness. Second, communication impacts preparedness. Community members speak different languages and often do not have proficiency in English. Access to resources in native languages and creative communication tactics are important tools. Finally, the study revealed a unique interplay between government, community-based organizations, and the refugee community. A web of formal and informal responses is vital to helping this community in times of need.

The refugee community in Clarkston, GA faces challenges, and disaster preparedness may not be top of mind for them. However, clear communication, disaster preparedness planning, and collaboration between government, community-based organizations, and the community are possible areas to focus on to bolster readiness.

The primary objective was to analyze the impact of the national cyberattack in May 2021 on patient flow and data quality in the Paediatric Emergency Department (ED), amid the SARS-CoV-2 (COVID-19) pandemic.

A single site retrospective time series analysis was conducted of three 6-week periods: before, during, and after the cyberattack outage. Initial emergent workflows are described. Analysis includes diagnoses, demographic context, key performance indicators, and the gradual return of information technology capability on ED performance. Data quality was compared using 10 data quality dimensions.

Patient visits totaled 13 390. During the system outage, patient experience times decreased significantly, from a median of 188 minutes (pre-cyberattack) down to 166 minutes, most notable for the period from registration to triage, and from clinician review to discharge (excluding admitted patients). Following system restoration, most timings increased. Data quality was significantly impacted, with data imperfections noted in 19.7% of data recorded during the system outage compared to 4.7% before and 5.1% after.

There was a reduction in patient experience time, but data quality suffered greatly. A hospital’s major emergency plan should include provisions for digital disasters that address essential data requirements and quality as well as maintaining patient flow.

To promote equity for intersectionally disaster-vulnerable individuals and address three literature gaps: (1) incremental effects of collective and self-efficacy as preparedness predictors, (2) differentiation of fear and perceived severity of a disaster, and (3) clarification of the relationship between fear and preparedness.

Due to infection risks associated with communal housing, early in the coronavirus disease (COVID-19) pandemic, many universities permitted students to remain in campus housing only if they were housing insecure, including many international students. We surveyed intersectionally-vulnerable students and their partners at a southeast US university, N = 54, who were international (77.8%), Asian (55.6%), and/or housing insecure at baseline (79.6%). In 14 waves from May–October 2020, we assessed pandemic preparedness/response behaviors (PPRBs) and potential PPRB predictors.

We examined within- and between-person effects of fear, perceived severity, collective efficacy, and self-efficacy on PPRBs. Within-person perceived severity and collective efficacy both significantly, positively predicted greater PPRBs. All effects of fear and self-efficacy were not significant.

Perceived severity and confidence that one’s actions positively impact one’s community fluctuated throughout the pandemic and are linked to greater PPRB engagement. Public health messages and interventions to improve PPRB may benefit from emphasizing collective efficacy and accuracy over fear.
