To assess the correlation between the dose distribution provided by the delivery analysis (DA) software and the measured dose distribution using an ArcCHECK (AC) phantom in the presence of thickness variation.

Two sets of targets were established within the phantom. Target A was placed on the detector areas, whereas Target B was positioned at the centre of the phantom. Bolus was applied to the surface of the phantom at different thicknesses ranging from 0 to 2 cm to verify the dose distribution in both TomoHelical (HT) and TomoDirect (TD) techniques. The gamma passing rate (GPRs) were evaluated against predefined thresholds of 3%/3 mm and 3%/2 mm. Correlation study evaluated the level of agreement between DA and AC values.

Both AC and DA exhibited a decline in GPRs as the bolus thickness decreased. Significant correlations were observed between AC and DA for both HT and TD techniques, with a p value of less than 0.001.

The results indicate that DA software has the capability to detect anatomical changes during tomotherapy treatments. This is substantiated by the statistical association found between DA and the standard AC phantom system for dose distributions in both HT and TD methods.

To limit the entrance dose to normal tissue and achieve the appropriate treatment time (TT) by using three different virtual structures with directional blocks for left-sided post-mastectomy radiation therapy (PMRT) with regional nodal irradiation (RNI).

Ten breast cancer patients who received PMRT by helical tomotherapy were enrolled. Three virtual structures were created for each patient: Organ-based, L-shaped (LB) and C-shaped (CB). The dose to the target and organ at risk (OARs), TT, the volume which received dose 5 Gy (V5Gy), integral dose (ID) and block structure contouring workload (BSCW) of the three virtual block techniques were evaluated. The performance scores were used to explore the suitable technique.

The CB plans showed a significantly better V5Gy, ID and contralateral breast-sparing. However, the CB plans revealed the longest TT and BSCW (p < 0·001). Contrary to the LB, the LB plans showed a significantly reduced TT and BSCW and provided the balance of plan efficiency with the highest score.

The LB technique is considered to be the suitable technique for left-sided PMRT with RNI and provided the advantage of TT, V5Gy, ID and BSCW while maintaining acceptable criteria for the target and OARs.

To evaluate efficacy of helical tomotherapy (HT) for treatment of breast cancer with internal mammary lymph node involvement.

This is a retrospective clinical audit of planning, dosimetry, toxicity and short-term survival of a cohort of 65 patients. Patients were treated between November 2014 and May 2019. The primary and nodal region was prescribed a dose of 50 Gray (Gy) in 25 fractions, while all cases of breast conserving surgery received a simultaneous integrated boost to a dose of 61 Gy in 25 fractions.

The 95% coverage for the primary, supraclavicular, internal mammary node and tumour bed was 93·4%, 96·8%, 90·7% and 98·3%, respectively. Mean dose to total lung, heart and contra-lateral breast was 10·6 Gy, 6·92 Gy and 4·32 Gy, respectively. None developed grade III skin or oesophageal toxicity. Twenty-one patients had progression; of which eighteen developed only distant failure while three also had loco-regional recurrence. At a median follow-up of 36 months, the 3-year loco-regional control, disease-free survival and overall survival were 93·5, 73·9 and 85·9%, respectively.

We report encouraging clinical outcome for patients treated uniformly with HT. The predominant pattern of failure was distant metastases which suggests the need for systemic control intensification.

Dosimetric comparison between volumetric-modulated arc therapy (VMAT) and helical tomotherapy (HT) in the treatment of bilateral breast cancer (BBC).

Ten patients treated on HT were selected retrospectively. Dose prescription was 50 Gy in 25 fractions to breast/chest wall and supraclavicular fossa (SCF) while tumour bed was simultaneously boosted to 61 Gy in 25 fractions. VMAT plans were made with four mono-isocentric partial arcs. The monitoring unit (MU) and treatment time were used to quantify the treatment efficiency. Target volumes were compared for homogeneity index (HI), conformity index (CI) while organs at risk (OARs) were compared for relevant dose volumes and integral doses (IDs).

For targets, no significant difference is observed between VMAT and HT in CI but VMAT could give better HI. The mean lung dose, V20 and V5 is 10·6 Gy versus 8·4 Gy (p-value 0·03), 12% versus 11·5% (p-value 0·5) and 78·1% versus 43·4% (p-value 0·005), respectively. The mean heart dose, V30 and V5 is 4·9 Gy versus 4·7 Gy (p-value 0·88), 0·5% versus 1·5% (p-value 0·18) and 26·2% versus 22·8% (p-value 0·4). Integral dose (ID) for the whole body and heart are comparable: 289 Gy kg versus 299 Gy kg (p-value 0·24) and 2·9 Gy kg versus 2·8 Gy kg (p-value 0·80). ID for lungs was significantly higher with VMAT: 7·9 Gy kg versus 6·3 Gy kg (p-value 0·03). There is a 53% reduction in treatment time and 78% in MU with VMAT against HT.

VMAT can generate clinically acceptable plans comparable to HT for BBC. HT shows better control over low dose spillage in lungs compared to VMAT thereby increasing ID to lungs. VMAT shows better homogeneity and efficient treatment delivery than HT.

Nasopharyngeal carcinoma (NPC) patients may have anatomical variations during their radiotherapy treatment course. In this study, we determine the daily accumulated dose by the deformable image registration (DIR) process for comparing with the planned dose and explore the number of fractions which the daily accumulated dose significantly changed from the planned dose.

The validation of the DIR process in MIM software has been tested. One hundred and sixty-five daily megavoltage computed tomography (MVCT) images of NPC patients who were treated by helical tomotherapy were exported to MIM software to determine the daily accumulated dose and then compared with the planned dose.

The MIM software illustrated the acceptable validation for clinical application. The accumulated dose (D50%) of the planning target volume (PTV70) showed a decrease from the planned dose with an average of 0.5 ± 0.27% at the end of the treatment and was significantly different from the planned dose after the second fraction of the treatment (p-value = 0.008). In contrast, the accumulated dose of organ at risk (OAR) tended to increase from the planned dose and was significantly different after the fifth fraction (left parotid), the twelfth fraction (right parotid) and the second fraction (spinal cord).

The inter-fractional anatomic changes cause the actual dose to be different from the planned dose. The dose differences and the number of fractions were varied in each target and OAR. The dose accumulation explored the necessary information for the radiation oncologist to consider adaptive treatment strategies to increase the efficiency of treatment.

The purpose of the present study is to compare hybrid [three-dimensional conformal radiation therapy-volumetric-modulated arc therapy (3DCRT-VMAT)] and helical tomotherapy (HT) techniques in terms of both planning target volume (PTV) and organs at risk (OARs) in the plans we made in locally advanced non-small cell lung cancer (NSCLC) patients

Radiotherapy was planned for 15 locally advanced NSCLC patients with 2 different techniques. Large tumours with positive mediastinal lymph nodes were preferred. The prescription dose was determined as 60 Gy at 30 fractions.

Mean PTV volume was 602·5 cc (range: 265–1461). Mean total lung volume was 4264 cc (range: 1885–6803). Homogeneity index, Dmean, Dmax, D2 and V105 were found to be lower in HT, V100, total monitor units (MU) and total beam on time were found to be lower in the hybrid plan. Total lung Dmean was found to be 17 Gy in both techniques. V10 value was 42·85 in the hybrid plan and 48·67 in HT (p = 0·037). Heart Dmean was 14·5 Gy in the hybrid plan and 18·7 in HT (p < 0·001), and V30 values were 18·1 and 22·9, respectively (p = 0·009).

Suitable dose coverage and OAR doses can be provided with both techniques. Especially the opposite lung, heart and oesophagus doses can be kept lower with the hybrid plan, and lower MU and shorter beam on time can be provided.

To evaluate changes of accumulated doses from an initial plan in each fraction by deformable image registration (DIR) with daily megavoltage computed tomography (MVCT) images from helical tomotherapy for prostate cancer patients.

The MVCT images of five prostate cancer patients were acquired by using a helical tomotherapy unit before the daily treatment fraction began. All images data were exported to DIR procedures by MIM software, in which the planned kilovoltage computed tomography (kVCT) images were acting as the source images with the daily MVCT acquired as the target images for registration. The automatic deformed structure was used to access the volume variation and daily dose accumulation to each structure. All dose-volume parameters were compared to the initial planned dose.

The actual median doses of the planning target volume (PTV) received 70 Gy and 50.4 Gy were decreased at the end of the treatment with an average 1·0 ± 0·67% and 2·1 ± 1·54%, respectively. As regards organs at risk (OARs), the bladder and rectum dose-volume parameters tended to increase from the initial plan. The high-dose regions of the bladder and rectum, however, were decreased from the initial plan at the end of the treatment.

The daily actual dose differs from the initial planned dose. The accumulated dose of target tends to be lower than the initial plan, but tends to be higher than the initial plan for the OARs. Therefore, inter-fractional anatomic changes should be considered by the DIR methods, which would be useful as clinically informative and beneficial for adaptive treatment strategies.

To assess helical tomotherapy (TOMO) current clinical application and practice in mainland China.

Data were collected for all TOMO units clinically operational in mainland China by 30 April 2016, including (a) the distribution of installation and staffing levels; (b) types of cancers treated; (c) utilisation efficiency; (d) quality assurance; (e) maintenance; (f) optional features; and (g) satisfaction levels. The data were collected as a census and analysed qualitatively and quantitatively.

As of 30 April 2016, 23 TOMO units were used clinically by 22 hospitals in mainland China. In the same period, 22,558 cancer patients were treated. For TOMO units with more than a year of clinical utilisation, a median of 378 cases were treated annually per machine. The median daily operation was 10·5 hours, and treatment headcount was 38·3 patients. The median service outage rate was 2·6%, and the most common cause was malfunction of the multi-leaf collimator. In terms of overall satisfaction levels, 3 hospitals were very satisfied, 16 were satisfied and 3 considered their satisfaction level as average.

The overall operation of TOMO is good, but there are some problems due to running at full capacity, lack of clinical efficacy research and insufficient quality assurance regulations.

Craniospinal irradiation (CSI) has become an important and challenging radiation technique for radiation oncologists. Helical tomotherapy (HT) seems to have dosimetric advantage for CSI compared with other radiation modalities. The purpose of this study was to compare dosimetric data between two different HT plans; simultaneous integrated boost (SIB) and sequential boost (Sq).

Twelve previously treated CSI contoured datasets by SIB technique were replanned. Dosimetric comparative parameters of targets were conformity index (CI) and homogeneity index (HI). For organ at risk (OARs), the mean dose of parallel organs, D2% of serial organs and whole body integral dose (ID) were also investigated.

SIB plan significantly provided more conformed dose to CSI and tumour boost while resulting in a similar CI in spinal boost region compared with Sq plan. The HI showed no differences between two plans. Radiation exposure to serial organs and ID were also significantly lower in SIB plan.

CSI treatment using HT, SIB technique was feasible and had more target coverage while minimising the radiation dose to healthy tissues.

To investigate possible optimisation of the image guidance procedure for the prostate cancer patients with respect to imaging frequency and patient body mass index (BMI).

The 6,085 setup correction shifts and BMI for 216 prostate cancer patients treated on tomotherapy units in two centres were analysed. Margins needed to account for inter-fraction target motion with daily only automatic correction and with automatic and manual corrections during one, three or five first fractions as a reference for further treatment without imaging were calculated.

The planning target volume margin calculated for the daily automatic correction only scheme was significantly lower than the margins calculated for the image guidance limited to a few initial fractions. Manual corrections after automatic fusion were more important for patients with higher BMI. On average, the patients with normal BMI had manual correction shift of 0·7 mm in anterioposterior direction, while overweight and obese patients required, correspondingly, the shifts of 1·3 and 1·4 mm.

Overweight and obese patients require daily imaging with time saving available by performing automatic kV/MV computed tomography registration only. The patients with normal BMI may be treated with imaging guidance during a few initial treatment fractions.
